REQUEST FOR CONSULTANT FOR BREAST CANCER DIAGNOSTICS
 
INTRODUCTION/BACKGROUND
Jhpiego is a non-profit organization affiliated with Johns Hopkins University, dedicated to improving health outcomes for women and families worldwide. By collaborating with local health experts and governments, Jhpiego develops sustainable solutions for healthcare systems. Their approach involves training healthcare workers, strengthening health systems, and providing innovative, low-cost solutions to ensure that even the most vulnerable populations receive the care they need.
 
Project Overview
The BEAT Breast Cancer initiative is a timely response to the increasing burden of breast cancer in Ghana. With funding from the Pfizer Foundation, Jhpiego’s collaboration with the Ministry of Health (MoH) and the Ghana Health Service (GHS) demonstrates a strong commitment to tackling breast cancer in a holistic and women-centered manner. This initiative focuses on awareness creation, early detection, timely diagnosis, and linkage to care to improve health outcomes for women in Ghana. 
 
Breast cancer is the most frequent cause of cancer death in women across Africa. In Ghana, breast cancer has the highest incidence and mortality of all cancers. Jhpiego is partnering with the Pfizer Foundation on Breast Cancer End-to-end care from Assessment to Treatment (BEAT) Breast Cancer in Ghana; a transformational initiative aimed at reducing mortality among women with Breast Cancer through stage-shifting, early diagnostics, and timely treatment. This partnership seeks to reduce the symptomatic period of breast cancer in Ghana from the current 12 months to less than three months by rooting the intervention in innovations, women-centered care, and patient navigation pathways to saturate PHC capacity for early detection by clinical breast exam (CBE), institutionalize breast health awareness interventions in social behavior change communications (SBCC) platforms, ensure timely diagnosis by decentralizing diagnoses at the secondary level, and facilitate access to multimodality treatment.
 
As part of activities to improve access to high-quality breast diagnostics at the PHC, this project aims to train and equip multidisciplinary health teams, including Pathologists, Radiologists, Radiographers, Sonographers, Medical Officers, and Medical Laboratory Scientists, with a broad range of skills required to drive these decentralization efforts.
To accomplish this, there is a need to develop a comprehensive in-service training module for breast screening assessment. In line with the above, a suitable Consultant is needed to deliver on this mandate. 
 
Objectives
The primary objective is to engage a consultant to develop structured and evidence-based training modules in breast cancer assessment covering:
  1. Tissue Sampling Techniques—including biopsy procedures: Fine Needle Aspiration (FNA), Core Needle biopsy, and open (Surgical) biopsy.
  2. Imaging/Ultrasonography for Breast Cancer – The principles, imaging techniques, interpretation, and correlation with pathology.
  3. Diagnostics: Tissue fixation and processing, histopathological evaluation, fine needle aspiration cytopathology (FNAC), immunohistochemistry (IHC), molecular diagnostics, and quality control.
  4. Clinical Breast Examination Guideline: Learning Guide for Clinical Breast Examination Guide
Terms of Reference (TOR)
The Consultant will be responsible for the following:
 
a. Development of Training Modules
Module 1: Tissue Sampling Techniques
  • Applied anatomy and physiology of the breast
  • Types of breast biopsies: Fine-needle aspiration (FNA), core needle biopsy, vacuum-assisted biopsy, and surgical biopsy.
  • Best practices in sample collection, handling, and fixation.
  • Clinical correlation and interpretation of biopsy results.
Module 2: Ultrasonography for Breast Cancer
  • Fundamentals of breast ultrasonography.
  • Sonographic features of benign and malignant breast lesions.
  • The role of ultrasound-guided biopsy techniques.
  • Image interpretation and correlation with histopathology.
Module 3: Laboratory Diagnostics
  • Tissue processing and staining.
  • Histopathological/Cytopathological assessment of breast cancer specimens.
  • Grading and staging of breast cancer.
  • Immunohistochemistry (IHC) and molecular markers (ER, PR, HER2, Ki-67).
  • Quality assurance in pathology and diagnostic reporting.
Module 4: Clinical Breast Examination
  • Anatomy and basic physiology of the breast
  • Clinical breast examination (CBE) procedure
  • Diagrammatic features accompanying CBE
  • The concept of Breast Screening and Management
  • Checklist for Clinical Breast Examination (CBE)
  • Counselling strategies for CBE
b. Training Delivery and Evaluation
  • Develop teaching materials, manuals, PowerPoint presentations, and case studies.
  • Conduct training sessions (in-person/virtual) for healthcare professionals, incorporating hands-on practical and simulation exercises in selected facilities.
  • Develop assessment tools to evaluate participant competency.
  • Provide recommendations for continuous professional development (CPD) and mentorship programs.
Reporting and Coordination
  • The Consultant will report to the Project Lead and work closely with radiologists, oncologists, and laboratory specialists.
  • Regular progress reports will be required at key milestones
  • All meeting proceedings shall be well documented and shared with stakeholders
  • Funding for the Consultancy shall be within the consideration of Jhpiego

Required Skills or Experience

Qualifications and Experience Required
  • Board-certified Pathologist/Surgeon/Radiographer with expertise in breast cancer diagnostics.
  • Experience in developing training curricula for healthcare professionals.
  • Strong knowledge of tissue sampling techniques, ultrasonography correlation, and laboratory diagnostics.
  • Experience in quality control and assurance in pathology laboratories.
  • Excellent communication and teaching skills.
